Breast Implants Under Vs Over The Muscle: How Placement Is Chosen

Breast implants can be positioned above the chest muscle, partly below it, or using a dual-plane technique. No placement is automatically best for every patient.

The surgeon chooses the implant position after examining breast tissue, skin quality, chest anatomy, lifestyle, and the desired result.

Where Can A Breast Implant Be Placed?

The breast sits over the pectoralis major muscle of the chest. During breast augmentation, the surgeon creates a pocket in a specific tissue layer and places the implant inside it.

The three commonly discussed positions are:

  • Over the muscle, also called subglandular placement
  • Under the muscle, usually referring to partial subpectoral placement
  • Dual-plane placement, which combines features of both positions

Patients considering zmadhojën e gjirit në Shqipëri should understand that placement and implant size must be planned together. Choosing one without considering the other can affect coverage, movement, and long-term support.

What Does Over-The-Muscle Placement Mean?

With over-the-muscle placement, the implant is positioned behind the natural breast tissue but in front of the pectoralis major muscle.

The breast tissue and skin cover the implant. The chest muscle remains behind it and is not lifted to form the main implant pocket.

This placement may be considered when the patient has enough natural tissue to conceal the implant. It can also be useful when avoiding implant movement during strong chest-muscle contraction is an important goal.

However, the implant may become more visible when the patient has thin tissue, limited breast volume, or weak skin support.

What Does Under-The-Muscle Placement Mean?

The phrase “under the muscle” can be slightly misleading. In many cosmetic breast operations, the implant is not completely covered by muscle.

The upper part of the implant is placed behind the pectoralis major muscle, while the lower part usually extends below its lower edge. This is often described as partial subpectoral placement.

The muscle provides additional coverage across the upper implant. This may create a smoother transition between the chest and breast, especially in patients with limited natural tissue.

The trade-off is that the implant can move when the chest muscle contracts. Recovery may also involve more muscle-related tightness or discomfort.

What Is Dual-Plane Breast Implant Placement?

Dual-plane placement combines muscle coverage in the upper breast with less muscle coverage over the lower implant.

The surgeon releases part of the connection between the muscle and breast tissue. This allows the implant to shape the lower breast while maintaining extra coverage toward the top.

There are different forms of dual-plane surgery. The amount of tissue separation and muscle release depends on the patient’s breast shape, nipple position, skin, and planned implant.

Dual-plane placement can be useful in selected patients, but it is not a universal solution. The surgeon must decide whether the additional tissue adjustment is appropriate.

Under Vs Over The Muscle: Main Differences

FaktorOver the MuscleUnder the Muscle
Implant positionBehind breast tissue and in front of the chest muscleAt least partly behind the chest muscle
Upper implant coverageDepends mainly on natural breast tissueAdditional coverage from the muscle
Muscle movementImplant is less affected by chest-muscle contractionImplant may move or change shape during contraction
Early discomfortMay involve less muscle-related discomfortMuscle tightness may be more noticeable
Visible implant edgesMore possible when tissue is thinMuscle may help conceal the upper implant edge
RipplingCan be more visible with limited tissue coverageUpper rippling may be less visible because of muscle coverage
Lower breast shapingImplant can directly fill the breast pocketDepends on muscle release and surgical technique
Athletic considerationsAvoids animation caused by the implant being behind the muscleStrong chest activity may cause visible implant movement
Suitable patientsOften patients with adequate tissue and skin supportOften patients who need more upper implant coverage

Tissue Thickness Is One Of The Most Important Factors

The surgeon must determine whether the patient has enough tissue to cover the implant.

A thin patient with very little breast tissue may have a greater chance of visible implant edges or rippling with over-the-muscle placement. Additional muscle coverage may make the upper part of the breast look smoother.

Patients with more natural breast tissue may have enough coverage for an over-the-muscle implant. Even then, tissue thickness should be evaluated in different areas because coverage may not be equal across the entire breast.

The surgeon may examine the thickness of the tissue by gently pinching it, taking measurements, and assessing the breast from several angles.

Skin Quality And Support Also Matter

Skin must support the weight of the breast and implant. Thin, stretched, or less elastic skin may not hold a large implant well, regardless of where it is placed.

Weak tissue can increase the possibility of:

  • Future breast stretching
  • A lower implant position
  • Visible implant edges
  • Rippling
  • Changes in the breast crease
  • Early return of sagging
  • Dallime midis dy anëve

Placing an implant under the muscle does not permanently protect weak skin. The lower breast still needs enough support, particularly when a larger implant is selected.

A moderate implant that fits the patient’s tissue may provide a more stable result than using extra volume to fill loose skin.

Implant Size, Width, And Profile Influence Placement

Implants should not be selected according to cup size alone. Bra sizes differ between manufacturers and do not provide an accurate surgical measurement.

The surgeon considers:

  • Chest width
  • Breast base width
  • Existing breast volume
  • Tissue thickness
  • Elasticiteti i lëkurës
  • Pozicioni i palosjes së gjirit
  • Pozicioni i thimthit
  • Distance between the breasts
  • Implant projection
  • Natural asymmetry
  • Long-term tissue support

A wide implant may extend beyond the patient’s natural breast dimensions. An implant with excessive projection may place unnecessary pressure on the skin.

Placement cannot correct an implant that does not fit the chest. Implant dimensions and pocket position must support the same surgical plan.

The Desired Appearance Is Part Of The Decision

Some patients want a softer transition from the chest to the breast. Others prefer more visible upper fullness.

Under-muscle or dual-plane placement may provide a gradual upper slope in patients with thin tissue. Over-muscle placement can sometimes create more direct fullness because the chest muscle does not press over the implant.

However, placement alone does not determine whether the result looks natural. The final appearance also depends on:

  • Implant volume
  • Implant profile and width
  • Natural breast tissue
  • Skin strength
  • Chest shape
  • Breast crease
  • Pozicioni i thimthit
  • Dallime midis dy anëve
  • The quality of the implant pocket

A natural-looking result comes from matching all these factors rather than following a single placement rule.

Previous Breast Surgery Can Change The Plan

Patients replacing existing implants require a different assessment from first-time augmentation patients.

The surgeon may need to evaluate:

  • The current implant position
  • Existing scar tissue
  • Kontraktura kapsulare
  • Tissue thickness
  • Lëvizje e implantit
  • Asimetri e gjirit
  • Changes in the breast crease
  • Skin stretching
  • Ruptura ose zbrazja e implantit
  • Previous incision and pocket

Gjatë operacionit breast implant replacement in Albania, the existing pocket may be adjusted or changed to another tissue layer. This decision depends on the reason for revision and the condition of the remaining tissue.

Changing from under to over the muscle, or from over to under, is possible in selected cases. It is not automatically simple and may require scar-tissue treatment, pocket repair, a lift, or additional internal support.

Breastfeeding And Nipple Sensation

Implant placement is only one factor that may influence breastfeeding and breast sensation.

Faktorë të tjerë të rëndësishëm përfshijnë:

  • Incision location
  • Amount of tissue dissection
  • Previous breast surgery
  • Natural glandular tissue
  • Changes to nerves or milk ducts
  • Healing response
  • Future pregnancy-related changes

No surgeon can guarantee that breastfeeding or nipple sensation will remain unchanged. Patients who plan to become pregnant should discuss this before surgery.

Pregnancy can also change breast volume, skin, and position, even when the implants remain intact.

Mammograms And Implant Monitoring

Breast implants do not remove the need for normal breast screening. Patients should tell the radiology team that they have implants when arranging a mammogram or another breast examination.

Special views may be needed to show more of the natural breast tissue. Screening recommendations should be based on age, medical history, implant type, symptoms, and local clinical guidance.

Patients should also know the manufacturer and model of their implants. Any new swelling, pain, change in shape, lump, or unusual firmness should be medically assessed.

Risks Of Breast Implant Surgery

Every breast implant operation has risks, regardless of placement.

Possible complications include:

  • Gjakderdhje
  • Infeksione
  • Shërimi i vonuar i plagëve
  • Mbledhje e lëngjeve
  • Changes in nipple or breast sensation
  • Cikatrice të dukshme ose të zgjeruara
  • Lëvizje e implantit
  • Asimetri e gjirit
  • Rippling
  • Kontraktura kapsulare
  • Ruptura ose zbrazja e implantit
  • Changes in the breast crease
  • Animation deformity
  • Persistent discomfort
  • Kompikimet e lidhura me anestezinë
  • Nevoja për kirurgji shtesë

Breast implants are not lifetime devices. Future surgery may be needed because of complications, tissue changes, implant damage, pregnancy, ageing, weight change, or personal preference.

Recovery After Under- Or Over-Muscle Implants

Recovery instructions depend on the pocket, implant, incision, and whether another procedure was performed.

Pyetjet E Bëra Më Shpesh

Is It Better To Have Breast Implants Under Or Over The Muscle?

Neither placement is better for every patient. The decision depends on tissue coverage, skin support, implant dimensions, chest-muscle activity, existing breast shape, and aesthetic goals.

Is Under-The-Muscle Placement Better For Thin Patients?

It may provide additional upper implant coverage when natural tissue is limited. However, the complete plan must also consider implant size, skin strength, lifestyle, and the possibility of muscle animation.

Can Over-The-Muscle Implants Look Natural?

Yes, when the patient has enough natural tissue and the implant fits the breast and chest. Thin coverage or an excessively large implant can make edges and rippling more visible.

Do Under-Muscle Implants Always Move During Exercise?

Not always. The amount of movement varies according to muscle strength, pocket design, surgical technique, and implant position.

Can Implant Placement Fix Sagging Breasts?

Placement alone cannot correct significant sagging or a low nipple position. A breast lift may be needed to remove excess skin and reshape the breast.

Can The Implant Pocket Be Changed Later?

It may be possible during revision surgery. The surgeon must assess scar tissue, skin quality, existing implant position, and whether pocket repair or a breast lift is required.
